From Precision Apparatus company, Inc 70-31- 84th Street, Glendale 27, L. L., N. Y. Has a center fold crease, at one edge next to the fold a 1/4 x 1/4 tear hole, at the same edge  several 1/4' nicks and crease, some center and side brown staining, some crease and tanning on the paper.
